Treatment of benign paroxysmal positioning vertigo of the horizontal canal, revisited

Abstract

After mentioning some important features of benign paroxysmal positional vertigo of the horizontal canal (HorBPPV), the therapy, which is essentially physical, is reviewed. The maneuvers by Baloh and Lempert are very similar and both have been performed with success. However, only two cases of each have been reported. The procedure by De la Meilleure et al. and the present variant have been applied in a more important group of patients, with promising results. With a forced prolonged position, Vannucchi et al. also obtained better results than by omitting treatment. These techniques have a common rationale, i.e. to move debris out of the horizontal semicircular canal. Finally, the classical Brandt-Daroff exercises are also worth trying for the horizontal type of BPPV. Medication, if necessary, is only additional.
