Abstract
Despite the advances in transnasal and external dacryocystorhinostomy, transsaccal dacryocystorhinostomy is an excellent addition to the armamentarium in chronic fistulizing dacryocystitis with a dilated tear sac in elderly patients. A large incision with excision of the fistula opens the lacrimal sac, from where a wide dacryocystorhinostomy can be opened into the nose. The skin and tear sac incision can be closed in one layer.
