Abstract
Two cases of cutaneous adiaspiromycosis by Emmonsia crescens are reported. This is the first human skin infection by this species and is the first report of its kind in man from India. In the first patient, the agent was demonstrated in KOH mounts, histology and culture from irregular, pigmented skin plaques on the right gluteal area. The lesion also contained calcium. In the second patient the fungus was demonstrated histologically in a knee lesion. The agent had elicited a histiocytic and giant cell reaction in the dermis in both cases. The first patient suffered from anaemia and epilepsy and the second suffered from nephropathy with chyluria. The skin lesions were surgically excised with skin grafting in the first patient.
Résumé
Deux cas d'Adiaspiromycose cutanée à Emmonsia crescens sont décrits. Il s'agit de la première infection de la peau par cette espèce; c'est aussi la première description de cette maladie chez l'homme en Inde. Chez le premier patient, l'agent responsable a été observé dans des préparations dans la potasse, des coupes d'histologie et des cultures faites à partir de taches cutanées irrégulières, pigmentées, situées sur l'aire fessière droite. La lésion contenait également du calcium. Chez le deuxieme patient, le champignon a été histologiquement démontré dans une lésion du genou. Dans les deux cas, l'agent responsable a provoqué une réaction du derme à histiocytes et cellules géantes. Le premier patient était atteint d'anémie et d'épilepsie et le second de néphropathie avec chylurie. L'excision chirurgicale des lésions cutanées du premier malade a été suivie de greffe de la peau.
