Growth of Argentinian girls with Turner syndrome

Summary

Growth data on 254 patients with Turner syndrome from Argentina—120 with XO karyotype and 134 with other chromosomal abnormalities—were analysed. Birth weight and height were significantly reduced. Ninety patients had received oestrogen treatment from a mean age of 14·0 years (SD 1·2) and 17 patients had spontaneous menarche. Patients who underwent spontaneous menarche had a small growth spurt. Final height was slightly higher (139·8 cm SD 5·6), though not significantly different from the mean adult height of the whole sample (137·9 cm SD 5·7). Mean adult height was 3·73 SD below mean of the normal local population. Mean height velocities from birth to maturity are very similar to those found in other samples. Distance standards were prepared by fitting a fifth-degree polynomial to the interpolated mean heights at each 0·5 year of age, and to the raw SD. Selected centiles were then calculated from the smoothed values. Differences between adult height in local Turner syndrome girls and local normal population are very similar to the same Turner-normal differences described in other communities. Standards presented here are useful for evaluating Turner syndrome patients from Argentina, and may also be used by those with similar growth pattern in their normal population.

Die Wachstumsdaten von 254 argentinischen Patientinnen mit Turner Syndrom—120 mit einem XO Karyotyp und 134 mit anderen chromosomalen Abweichungen—wurden analysiert. Das Geburtsgewicht und die Geburtslänge waren signifikant reduziert. 90 Patientinnen hatten durchschnittlich ab dem Alter von 14 Jahren (SD 1·2 Jahre) eine Östrogentherapie erhalten und 17 hatten eine spontane Menarche. Patientinnen mit spontaner Menarche ließen einen geringen Wachstumsspurt erkennen. Die Endhöhe war geringfügig höher (139·8 cm, SD 5·6) allerdings nicht signifikant verschieden von der mittleren Erwachsenenhöhe der Gesamtstichprobe (137·9 cm, SD 5·7). Die durchschnittliche Erwachsenenhöhe lag 3·73 Standardabweichungen unter dem Mittel für die gesunde lokale Bevölkerung. Die durchschnittliche Wachstumsgeschwindigkeit der Körperhöhe von der Geburt bis zur Reife ist der in anderen Stichproben beobachteten sehr ähnlich. Durch Anpassung eines Polynoms fünften Grades an die interpolierten durchschnittlichen Körperhöhen für Halbjahre und der rohen Standardabweichungen wurden Distanzkurven entwickelt. Aus den geglätteten Werten wurden dann ausgewählte Perzentile abgeleitet. Die Differenz zwischen der Erwachsenenhöhe der Turner-Patientinnen und der gesunden weiblichen Vergleichsbevölkerung war entsprechenden Unterschieden in anderen Bevölkerungen ähnlich. Die mitgeteilten Standards sind zur Beurteilung des Wachstums von argentinischen Turner-Patientinnen nützlich. Sie können auch für Patientinnen aus Bevölkerungen mit ähnlichen Wachstum in der Normalbevölkerung herangezogen werden.

Résumé

On a analysé des données de croissance concernant 254 patients argentins présentant le syndrome de Turner (120 avec des caryotypes XO et 134 avec d'autres anomalies chromosomiques). Le poids à la naissance et la stature sont significativement réduits. 90 patients ont reçu un traitement d'oestrogène à partir d'un àge moyen de 14·0 ans (ET 1·2) et 17 ont présenté des premières règles spontanées. Ces derniers ont manifesté en outre une légère accélération de croissance et leur stature finale (139·8 cm ET 5·6) était légèrement plus élevée bien que non significativement différente de la stature moyenne adulte de l'ensemble de l'échantillon (137·9 cm ET 5·7). La stature moyenne adulte est de 3·73 ET en-dessous de la moyenne normale de la population locale. Les vélocités moyennes de croissance staturale de la naissance jusqu'à la maturité, sont similaires à celles trouvées dans d'autres échantillons. Les standarts de distance ont été préparés en ajustant un polynome du cinquième degré, aux statures moyennes interpolées pour chaque demi-année d'àge et aux écart-types bruts. Les centiles choisis ont alors été calculés à partir des valeurs lissées. Les différences des statures adultes entre les filles locales atteintes du syndrome de Turner et la population locale normale, sont très semblables aux différences Turner-normal décrites dans d'autres communautés. Les standarts présentés ici sont utiles pour évaluer les patients atteints du syndrome de Turner en Argentine aussi bien que pour les enfants normaux dont les modalités de croissance correspondent à la population normale étudiée.
