Vestibular disorders in children

Abstract

When vertigo and dizziness are diagnosed in a child, this is a matter of concern for the physician and the child's family. Prior to a comprehensive otological, neurological, and vestibular clinical examination, physicians often request CT or MRI scanning, although in most cases such expensive testing is unnecessary. The present work is based on the results of a 14-year study conducted with a sample of more than 2,000 children referred for vertigo and balance disorders to the functional vestibular evaluation unit of the ENT pediatric department at Robert Debré Hospital (Paris). The clinical signs of vestibular deficit and the most frequent aetiologies of vertigo and dizziness in children are detailed (migraine equivalent, ophthalmologic disorders, benign paroxysmal idiopathic pediatric vertigo, and temporal bone fracture), which has been helpful in determining the best diagnostic procedure and therapy for vertigo in children.

El diagnóstico de vértigo y mareo en un niño es motivo de preocupación para el médico y para la familia del niño. Antes de un examen integral otológico, neurológico y clínico vestibular, los médicos frecuentemente solicitan CT o MRI a pesar de que en muchos casos estas costosas evaluaciones son innecesarias. Este trabajo se basa en los resultados de un estudio de 14 años que se llevó a cabo con una muestra de más de 2,000 niños referidos por vértigo o problemas de equilibrio, a la unidad de evaluación funcional vestibular del departamento de ORL pediátrica del Hospital Robert Debré de Paris. Se detallan los signos clínicos del déficit vestibular y de las etiologías más frecuentes de vértigo y mareo en niños (equivalente de la migraña, trastornos oftalmológicos, vértigo paroxístico idiopático benigno infantil y fractura de hueso temporal) que han sido útiles para determinar los mejores procedimientos diagnósticos y la terapia del vértigo en niños.
