Abstract
The term breech trial results published in 2000 concluded that women with singleton breech presentation at term should undergo elective caesarean section. Our aim was to analyse the mode of delivery of the second twin and ascertain the impact of results of the term breech trial on delivery of the non-vertex second twin. This retrospective analysis includes 103 twin deliveries between January 1998 and June 2002. Caesarean section before labour was carried out in 54% (n = 56) of women. Following delivery of the first twin, 60% of the second twin presented as cephalic and 38% were non-vertex. There was one stillbirth recognised following delivery of the first twin. Mode of delivery of the non-vertex second twin was breech extraction in 28%, assisted breech delivery in 39% and emergency caesarean section 33%. Internal podalic version was carried out successfully in four women and external cephalic version was unsuccessful in three cases. All breech extractions were carried out during 1998–99. The assisted breech deliveries in 1998–99 were 43% compared to 29% in 2001. There were 83% emergency caesareans during 2000–02, compared to 17% in 1998–99.
