The mode of action of fenfluramine and its derivatives and their effects on glycerolipid metabolism

Summary

The evidence is reviewed that fenfluramine and benfluorex could have both direct and indirect effects in decreasing the rate of triacylglycerol synthesis in the liver. The direct effect is mediated through the inhibition of phosphatidate phosphohydrolase and by the recycling of excess phosphatidate back to glycerol phosphate. The indirect effect involves alteration of the concentrations of glucocorticoids in the blood and this was demonstrated with rats treated chronically with benfluorex. In these animals, the extent of ethanol-induced increases in serum corticosterone, in hepatic phosphatidate phosphohydrolase activity, and in the synthesis and accumulation of triacylglycerol in the liver was diminished. It is also suggested that glucocorticoids are responsible for the increased synthesis of hepatic triacylglycerols that is seen in diabetes and in stress conditions, and after eating fructose, glycerol, sorbitol and saturated fat. A combination of these direct and indirect effects could contribute to the anti-obesity properties of fenfluramine and the hypolipidaemic effects of benfluorex.

Les études montrant que la fenfluramine et le benfluorex pourraient avoir des effets tant directs qu'indirects sur la diminution du taux de synthèse du triacylglycerol dufoie ont été analysés. l'effet direct se traduit par une inhibition de la phosphatidate phosphohydrolase et par le retour au glycerol phosphate de l'excès de phosphatidate. l'effet indirect comprend l'altération des concentrations de glucocortïcoldes dans le sang, ce qui a été démontri chez des rats traités de facon chronique au benfluorex. Chez ces derniers, l'importance de l'augmentation, due à l'éthanol, de la corticostérone sérique, de l'activité” de la phosphatidate phosphohydrolase hépatique, et de la synthése et de l'accumulation du triacylglycérol dufoie a diminué. II est aussipermis de penser que les glucocorticoïdes sont les responsables de Vaugmentation de la synthèse des triacylglycérols hépatiques que l'on remarque dans le diabète et dans les conditions de stress, ainsi qu'après l'absorption de fructose, de glycérol, de sorbitol et d'acides gras saturds. Une combinaison de ces effets directs et indirects pourrait étre une contribution aux proprUtes anti-obésité” de la fenfluramine et aux effets hypolipidémiques du benfluorex.
