Guidelines for Compounding Safe and Uniform Sustained Release Capsules

Abstract

The development of high-grade hydroxypropyl methylcellulose in the past decade made it possible for individual pharmacists to easily compound capsules that purport to have sustained-release properties. These capsules allow individualized doses and less frequent dosing. But these dosage forms may be harmful to patients if they are not designed and compounded properly. In order to ensure the homogeneity of the capsules, pharmacists must review the physicochemical properties and quality of each capsule ingredient and determine which cellulose derivative and fillers to use. They also must determine appropriate proportions of constituents to ensure sustained-release from the dosage form. After the pharmacist designs the appropriate capsule formulation, the next step is to prepare the sustained release capsules using proper weighing and mixing technique to ensure product continuity. In order to ensure batch to batch uniformity, the source and proportion of specific chemicals used, compounding techniques, and product labeling should be thoroughly documented.
