Modelling and simulation for planning and benefit/cost analysis of health-care service systems

Abstract

This paper presents a new methodology for planning and optimizing laboratory activities, and for benefit/cost analysis, based on basic systems analysis and modelling of the different laboratories, each of which represents a sub-system of the health care system.

The models have been given a hierarchical structure, and can therefore be used in studies and problem solving concerning the planning of the laboratory function as a whole, as well as of different partial activities inside a laboratory department.

For a number of cases, the models have been used for computer simulations in order to study the different problems concerning the internal effectiveness of the laboratory.

To be able to study the external effectiveness of the laboratory, i.e. problems concerning the interactions between the patient, the medical decision process and the laboratory process, it is necessary to create a model describing the decision process in this respect. Through the use of such a model, it will be possible to evaluate the benefit of different laboratory structures.

Les laboratoires doivent faire face à une augmentation de leur charge de travail et doivent répondre aux exigences sur la qualité et les délais des résultats. Ces contraintes, auxquelles il faut ajouter le coût sans cesse croissant des soins, rendent nécessaire la planification et l'optimisation de leurs activités, et l'analyse de. leur coût-bénéfice.

Une nouvelle méthodologie, fondée sur l'analyse des systèmes, a été appliqué a la modélisation des différents types de laboratoires, où chacun représente un sous-système au sein d'un grand système de soins. Ces modèles ont cmduit des structures hiérarchisées. 11s peuvent ětre utilisés pour optimiser par simulation le fonctionnement global du laboratoire ou l'une seulement de ses activités. Deux questions ont fait l'objet d'une étude particulière: l'allocation des ressources et le comportement du laboratoire en fonction des examens urgents et des délais exigés pour les réponses.
