Shoulder-Hand Syndrome and Herpes Zoster: Report of a Case with Signs of Vasodilatation and Vasoconstriction in the Same Hand

Summary

A case report is given of a woman of 65, who developed first a painful shoulder, then a herpes zoster brachialis of the 6th and perhaps the 5th cervical root, and finally a reflex dystrophy of the hand of the right superior extremity. An osteoarthritis of the cervical spine probably was the primary lesion. A partial remission took place in 8 months. During the acute stage of the disease there was evidence of vasospasm in the thumb, the cutaneous sensibility of which was impaired, and of vasodilatation of the fingers. Two years later the whole hand was found to be colder than the healthy one. In the discussion the case is seen in the light of the hypotheses of Livingston and de Takats concerning vasomotor disturbances in reflex dystrophy.

L'auteur décrit le cas d'une femme de 65 ans qui fut d'abord atteinte de douleurs dans une épaule, ensuite d'un zona brachial de la 6e et peutêtre la 5e racine cervicale, enfin d'une dystrophie réflective de la main droite. La première lésion était probablement caractérisée par une ostéoarthrite de l'épine cervicale. Une rémission partielle survint après le 8e mois. Durant la phase aigue de la maladie l'auteur observa des spasmes vasculaires dans le pouce dont la sensibilité cutanée était troublée ainsi qu'une vasodilatation des doigts. Deux ans plus tard il constata que la main entière était plus froide que la main saine. Dans la discussion du cas celui-ci est considéré à la lumière des hypothèses de Livingston et de de Takats concernant les troubles vasomoteurs dans la dystrophie réflective.

Zusammenfassung

Es wird ein Bericht vorgelegt über den Fall einer 65 Jahre alten Frau, bei der zuerst Schmerzen in der Schulter auftraten, dann eine Oberarm-Gürtelrose der 6. und vielleicht der 5. Halswurzel und schliesslich eine reflektorische Dystrophie der rechten Hand. Eine Knochen- und Gelenkentzündung der Halswirbelsäule war wahrscheinlich die erste Störung. Eine teilweise Besserung war nach 8 Monaten eingetreten. Während des akuten Stadiums der Krankheit wurden Gefässkrämpfe im Daumen beobachtet, dessen Haut-Sensibilität gestört war, sowie Blutgefässerweiterung in den Fingern. Zwei Jahre später wurde festgestellt, dass die ganze Hand kälter war als die gesunde. Bei der Diskussion des Falles wird dieser im Lichte der Hypothesen von Livingston und de Takats gesehen, die vasomotorische Störungen mit reflektorischer Dystrophie in Beziehung bringen.
