Summary
An attempt was made to elucidate the relative importance of heredity and exogenous factors in the etiology of rheumatoid arthritis by a study of the concordance rate in monozygotic twins. Four sets of such monozygotic twins are reported and discussed. Three of them were discordant and one concordant, but with an atypical syndrome. When added to previously published cases in monozygotic twins, these cases bring the number up to 50 sets of monozygotic twins with rheumatoid arthritis 32 or 64 % of whom have shown discordance. It is concluded that an exogenous factor must be operative.

L'auteur fait un essai pour élucider l'importance relative de l'hérédité et des facteurs exogènes dans l'étiologie de la polyarthrite chronique évolutive en étudiant le degré de concordance dans des jumeaux monozygotiques. Quatre couples de pareils jumeaux monozygotiques ont fait l'objet d'un rapport et d'une discussion. Trois d'entre eux étaient discordants et le quatrième concordant, mais avec un syndrome atypique. En ajoutant ces cas à d'autres jumeaux monozygotiques étudiés précédemment, le nombre de jumeaux monozygotiques souffrants de polyarthrite chronique évolutive s'élève à 50 couples dont 32 ou 64 % ont montré une discordance. L'auteur conclut qu'un facteur exogène doit avoir été opératif.
Zusammenfassung
Es ist der Versuch gemacht worden, die relative Bedeutung von erblichen und exogenen Faktoren bei der ätiologie des chronischen Gelenkrheumatismus durch eine Untersuchung des Grades der Konkordanz bei monozygotischen Zwillingen zu erklären. Es wird über vier Paare solcher monozygotischer Zwillinge berichtet und die Resultate werden diskutiert. Drei dieser Paare waren diskordant und eins konkordant, jedoch mit einem atypischen Syndrom. Wenn man diese Fälle zu den bereits früher veröffentlichten Fällen von monozygotischen Zwillingen hinzuzählt, so steigt die Anzahl von monozygotischen Zwillingen mit chronischem Gelenkrheumatismus auf 50 Paare an, von denen 32 bezw. 64 % Diskordanz gezeigt haben. Es wird der Schluss gezogen, dass ein exogener Faktor hier wirksam gewesen sein muss.
