How variability in hymenoplasty recommendations leads to contrasting rates of surgery in the Netherlands: an ethnographic qualitative analysis

Abstract

Hymenoplasty is surgery to alter the shape of the hymen membrane in the vaginal canal, commonly performed to minimise the aperture. This medical operation is often requested by women who expect that their virginity will be under scrutiny, particularly during their first sexual encounter on their wedding night. Despite increasing demand for the surgery all over the globe, there is no one standard of practice in performing hymenoplasty. In the Netherlands, the manner in which medical consultations concerning the procedure take place depends heavily on the consulting physician. This paper looks at two different approaches to hymenoplasty consultation in the Netherlands: a pedagogical philosophy adopted in a public hospital and a practical approach employed by a private clinic. Each approach culminates in a contrasting result: patients in one medical establishment are twice as likely to undergo hymenoplasty than those visiting the other.

Résumé

L’hyménoplastie est la procédure chirurgicale qui permet de modifier la forme de l’hymen dans le canal vaginal, afin d’en réduire l’ouverture. Cet acte médical est souvent demandé par des femmes qui s’attendent à ce que leur virginité soit examinée, en particulier lors de leur première rencontre sexuelle durant leur nuit de noces. Malgré une demande croissante à travers le monde, il n’existe pas de procédure standard pour la réalisation de l’hyménoplastie. Aux Pays-Bas, la manière dont les consultations concernant l’hyménoplastie sont conduites dépend fortement des médecins. Cet article examine deux approches différentes de la consultation pour une hyménoplastie aux Pays-Bas : une philosophie pédagogique adoptée par un hôpital public et une approche pratique adoptée par un centre de soins privé. Chacune de ces approches conduit à des résultats opposés : les patientes qui consultent dans l’un des établissements ont deux fois plus de chances de subir une hyménoplastie que celles qui consultent dans l’autre.

Resumen

La himenoplastia es la cirugía para reconstruir la membrana del himen en el canal de la vagina, que normalmente se realiza para minimizar la apertura. Muchas mujeres solicitan esta operación médica cuando saben que se controlará su virginidad, especialmente durante la primera experiencia sexual en la noche de bodas. Pese a la creciente demanda de la himenoplastia en todo el mundo, no hay una práctica estándar para realizar este tipo de intervención. En los Países Bajos, la forma en la que se lleva a cabo la consulta médica sobre esta operación depende en gran medida del médico consultado. En este artículo se analizan dos enfoques diferentes en las consultas sobre la himenoplastia en los Países Bajos: una filosofía pedagógica adoptada en un hospital público y un enfoque práctico empleado por una clínica privada. Cada enfoque produce un resultado contrastante: es dos veces más probable que las pacientes en uno de los centros médicos se sometan a una himenoplastia que las que van al otro.
