Water irrigation versus air insufflation: A comparison of two caloric test protocols

Irrigación con agua versus insuflación con aire: comparación de dos protocolos de pruebas calóricas

Abstract

The aim of the present study was to construct and compare two caloric test protocols, one for water irrigation, and one for air insufflation. A set of reference data was constructed and tabulated as well as the intersubject variability. The effect of age, sex, ear, and temperature, as well as a possible priming effect and order effect were investigated. Forty-seven subjects (18–58 years) without otological or vestibular history participated. Four response parameters were investigated: slow component velocity (SCV), frequency, unilateral weakness (UW), and directional preponderance (DP). Statistically higher SCV values were obtained for water compared to air, with statistically higher standard deviations for SCV water values. No influences of age, sex, ear, or temperature could be demonstrated on any of the response parameters. The same applied for the presence of an order effect and a priming effect. Comparing the two protocols to one another led the present authors to favour water as the standard irrigation medium, and air only in situations where water is contra-indicated.

Sumario

El propósito de este estudio fue estructurar y comparar dos protocolos de pruebas calóricas, uno por irrigación con agua y otro por insuflación con aire. Se estructuró y tabuló una serie de datos de referencia además de la variabilidad entre sujetos. Se investigaron los efectos de edad, sexo, oído y temperatura, así como un posible efecto inicial y uno de orden. Participaron 47 Sujetos (de 18 a 58 años) sin antecedentes otológicos o vestibulares. Se buscaron 4 parámetros de respuesta: velocidad del componente lento (SVC), frecuencia, debilidad unilateral (UW) y preponderancia direccional (DP). Se obtuvieron valores estadísticamente mayores con el agua comparados con el aire y desviaciones estándar estadísticamente mayores con los valores SCV con el agua. No se pudo demostrar influencia de edad, sexo, oído o temperatura en ninguno de los parámetros de respuesta. Lo mismo sucedió aplicado a la presencia de un efecto de orden o a uno primario. La comparación de los dos protocolos, permite a los autores favorecer el agua como el medio estándar de irrigación y el aire solamente en situaciones en las que el agua está contraindicada.
