DRUG USERS AND HIV-COMBINATION THERAPY (HAART): FACTORS WHICH IMPEDE OR FACILITATE ADHERENCE

Abstract

The current highly active antiretroviral therapy (HAART), i.e., triple combination treatment including a protease inhibitor, allows for long-term suppression of HIV-replication. The intake guidelines, however, are very strict and, like many other HIV positive patients, drug users have problems in achieving good therapy compliance. As a result, the effectiveness of the medication therapy is seriously jeopardized. This qualitative study, involving in-depth interviews with 27 “hard drug” users in Amsterdam, seeks to provide better insights into the extent of adherence to HAART. In addition, factors that impede or facilitate adherence have been listed in order to develop and improve strategies. The study shows that almost half of the “hard drug” users interviewed do not adhere to the therapy. A large number of determinants for adherence were identified at the level of medication, the individual, and the environment. Many of these determinants can be influenced, which means that they can provide clues for the effective implementation of measures that may enhance effectiveness of treatment.

RESUMEN

La actual terapia antiretroviral de alta actividad (HAART) consiste en un tratamiento de combinación triple que incluye un inhibidor de la proteasa y permite reprimir la replicación del VIH por un periodo prolongado. Sin embargo, las instrucciones para la ingestión son muy estrictas y—como sucede con otros portadores del VIH—a los consumidores de drogas les cuesta mucho atenerse a las reglas propias de esta terapia, debido a lo cual la eficacia de este tratamiento médico corre grave peligro. Este estudio cualitativo, que comprende entrevistas en profundidad con 27 consumidores de drogas duras en Amsterdam, tiene como objetivo dar una idea más clara acerca del nivel de adherencia a HAART. Además se han enumerado los factores que impiden y los que facilitan la adherencia con el fin de crear y mejorar estrategias. El estudio demuestra que casi la mitad de los entrevistados que consumen drogas duras no sigue con la terapia. Muchos factores que determinan la adherencia se han identificado en el nivel del medicamento, del individuo y su ambiente. Es posible ejercer influencia sobre muchos factores determinantes, lo que significa que éstos ofrecen pistas para poner en práctica en forma eficaz las medidas que mejoren la terapia.

RÉSUMÉ

Les traitements anti-rétroviraux efficaces (HAART en anglais), à savoir les associations médicamenteuses contenant un inhibiteur de la protéase, permettent une suppression prolongée de la réplication du VIH. Cependant, les recommandations concernant les prises médicamenteuses sont très strictes et, comme beaucoup d’autres patients infectés par le VIH, les toxicomanes ont des difficultés à maintenir une bonne adhérence au traitement. Il en résulte une diminution importante de l’efficacité réelle du traitement. Cette étude qualitative, qui comprend un interrogatoire approfondi de 27 usagers de drogues dures à Amsterdam, vise à fournir une meilleure connaissance du degré d’adhérence au traitement (HAART) de ces sujets. De plus, les facteurs qui facilitent ou limitent l’adhérence ont été identifiés afin de développer ou améliorer les stratégies de mise à disposition et d’utilisation des traitements. L’étude montre que presque la moitié des usagers de drogues dures interrogés n’adhèrent pas à leur traitement. Un nombre important de facteurs déterminants de l’adhérence ont été identifiés au niveau des médicaments, des individus, ou de l’environnement. Nombre de ces facteurs sont modifiables, suggérant des approches plus performantes pour la mise à disposition et l’utilisation des traitements afin d’en améliororer l’efficacité.

Erik J. C. van Ameijden

Erik van Ameijden has worked for 9 years at the Amsterdam Cohort Studies on AIDS among drugs users and homosexual men at the Municipal Health Service in Amsterdam. His Ph.D. was on evaluation of AIDS prevention measures among drug users. He then worked for 2 years at the Trimbos Institute (National Institute of Mental Health and Addiction) as head of Addiction Epidemiology. Currently he is head of Epidemiology and Information at the Municipal Health Service in Utrecht.
