Following publication of this article, two errors have been identified by the authors in the notation of units.

1) page 662 line 24. The unit of white blood cell count should read 10⁶/L, and not 10⁹/L

2) page 662 line 26. The unit of platelet should read 10¹⁰/L, and not 10⁹/L
