Changes in gait following continuous intrathecal baclofen infusion in ambulant children and young adults with cerebral palsy

Abstract

Objective: To assess the effect of continuous intrathecal baclofen infusion (ITB) on gait parameters of ambulant children with cerebral palsy (CP).

Methods: The assessment before and 16 months after ITB on seven children with CP (mean age 15 years, SD 5.4) included: Ashworth scale score, range of motion (hip, knee, ankle), Gillette functional assessment questionnaire (FAQ), joint kinematics, spatiotemporal parameters and Gillette Gait Index (GGI).

Results: Gillette FAQ significantly improved from 6.1 to 7.1. Mean GGI improved from 554.50 to 489.25, which was not significant although the improvement was large for three children. Parameters in the sagittal plane were significantly modified with a significant increase in step length (0.65 m to 0.74 m) and a significant increase in hip extension during stance phase (32.25° to 21.6°). These results were correlated to clinical assessments.

Conclusion: ITB seems to improve sagittal gait parameters of children and young adults with CP.

Resumen

Objetivo: Valorar los efectos de la infusión continua de baclofeno intratecal (ITB) en los parámetros de la marcha de niños ambulatorios con parálisis cerebral (CP).

Métodos: La valoración antes y 16 meses después de la aplicación de infusión contínua intratecal de baclofeno en 7 niños con parálisis cerebral (edad media 15 años, DE 5.4) incluyó: la calificación en la escala de Ashworth, la amplitud del movimiento (cadera, rodilla, tobillo), el cuestionario de valoración funcional de Gillette (FAQ), cinemática articular, los parámetros espacio-temporales y el índice Gillette de la marcha (GGI).

Resultados: El cuestionario Gillete mejoró significativamente de 6.1 a 7.1. El GGI medio mejoró de 554.50 a 489.25, lo cual no fue significativo a pesar de que la mejoría fue importante para 3 niños. Los parámetros de la marcha en el plano sagital se modificaron significativamente, con un aumento importante en la longitud del paso (0.65 a 0.74 m) y un aumento en la extensión de la cadera durante la fase de apoyo (32.25° a 21.6°). Estos resultados se correlacionan con las valoraciones clínicas.

Conclusión: El baclofeno intratecal mejora los parámetros sagitales de la marcha en niños y adultos jóvenes con parálisis cerebral.
