The Impact of Having a Loved One With Alcohol Consumption-Related Problems on Subjective Health Status and Health-Risk Behaviors in a General Hospital Sample

Abstract

Consecutively admitted inpatients of a general hospital in Northern Germany (N = 2903) completed a standardized screening questionnaire in 2005–2006 containing questions about alcohol consumption-related problems in loved ones, their own alcohol consumption, and other health-related aspects. Almost 26.2% reported having a loved one with alcohol consumption-related problems. The proportions of smokers and those who screened positively for at-risk consumption or alcohol abuse were significantly higher in those patients; they were significantly younger and reported worse overall states of health and emotional health. Since those patients showed a high risk of numerous health problems, psychological interventions were implemented. The study's limitation noted.

RÉSUMÉ

L’influence d’avoir un membre de la famille alcoolique sur l’état de santé subjectif et les conduites à risques pour la santé selon une enquête réalisée dans un hôpital général

Tous les patients nouvellement enregistrés dans un hôpital général du nord de l’Allemagne (N = 2903) ont reçu un questionnaire de dépistage standard avec des questions portant sur les problèmes d’alcool dans la famille, sur la propre consommation d’alcool et d’autres aspects en matière de santé. 26,2% ont indiqué avoir un membre de la famille ayant des problèmes d’alcool. Parmi ces patients, il y avait nettement plus de fumeurs et de personnes avec un résultat de dépistage positif pour une consommation d’alcool à risque ou pour un abus d’alcool. En outre, ils étaient nettement plus jeunes et mentionnaient un état de santé mental et général plus mauvais. Étant donné que ces patients présentent un risque élevé par rapport à différents problèmes de santé, des interventions psychologiques doivent être introduites.

Los pacientes admitidos en un hospital general en el norte de Alemania (N = 2903) obtuvieron en 2005/06 un cuestionario estándar de verificación sobre antecedentes de alcoholismo en familiares, consumo de alcohol propio y otros aspectos relacionados. Un 26,2% reconoció tener algún familiar con problemas de alcoholismo. Entre estos pacientes se encontró un número significativamente mayor de fumadores y de personas que dieron un resultado de screening positivo a un consumo de riesgo o abuso del alcohol. Además, eran significativamente más jóvenes y declararon un peor estado de salud tanto general como anímico. Dado que estos pacientes mostraron un mayor riesgo de padecer distintos problemas de salud, deberían implementarse intervenciones psicológicas.

Gallus Bischof, PhD, Dipl. Psych., is presently working as a senior researcher in the Department of Psychiatry and Psychotherapy, University of Luebeck. Currently he is the principal investigator of a project on the effectiveness of CRAFT in Germany. Dr. Bischof's earlier research included natural recovery from alcohol dependence and evaluation of various approaches on early intervention for substance use disorders.

Susa Reinhardt, PhD, Dipl. Psych., is presently working as a clinical psychotherapist in Schwerin. Between 2001 and 2007 (degree) she was a junior researcher at the University of Luebeck in the field of early interventions for alcohol misuse.

Janina Grothues, PhD, Dipl. Psych., is presently working as a senior researcher in the Department of Psychiatry and Psychotherapy, University of Luebeck, and as a clinical psychotherapist in the field of addiction at the AHG Clinic, Holstein, Luebeck. Dr. Grothues's earlier research included early interventions for substance misuse, i.e., alcohol use disorders and smoking.

Ulfert Hapke, PhD, Dipl. Psych., Head of the Mental Health Research Group, Department of Epidemiology and Health Monitoring, Robert Koch Institute is a Federal Institute within the portfolio of the Federal Ministry of Health.

Prof. Dr. Ulrich John is the director of the Institute of Epidemiology and Social Medicine, Medical Faculty, University Greifswald since 1997, and head of the research group S:TEP (Substance Abuse: Treatment, Epidemiology and Prevention). He conducts research in the field of substance abuse and dependence, as well as municipal health care and the development of community medicine approaches. In 2000 he received the Wilhelm Feuerlein Award for research in alcoholism.

Jennis Freyer-Adam is a psychologist and a senior researcher. As part of the Research Collaboration Early Intervention in health risk behaviors (EARLINT) she conducts research projects in the field of development of brief interventions for risky alcohol drinkers.

Hans-Jürgen Rumpf, PhD, is working in the Department of Psychiatry and Psychotherapy, University of Luebeck. He is head of the research group S:TEP. His research covers studies on analytical epidemiology of substance-related disorders and gambling, as well as research on brief interventions in the field of alcohol and tobacco.
