Vitreous Humor/Blood and Urine/Blood Alcohol Ratios as an aid to the Forensic Investigator Part 2: Application

ABSTRACT

Post-mortem blood alcohol concentrations were compared to corresponding levels in vitreous humor and urine. Case study reports from 182 cases were examined for the effect of recent drinking and the state of consciousness prior to death on the vitreous humor/blood alcohol ratio (VH/BL), and on the urine/blood alcohol ratio (UR/BL). The mean values and the standard deviations for three different categories of forensic significance for VH/BL were: no drinking/conscious (1.26 ± 0.11); no drinking/unconscious (1.46 ± 0.14); drinking/conscious (0.91 ± 0.17). The category of “no drinking” refers to a subject who was probably in a post-absorptive state prior to the incident. The UR/BL alcohol ratios tended to support the interpretation of the VH/BL results. The VH/BL alcohol ratios may provide the investigator with useful information concerning the drinking history and the state of consciousness of the deceased prior to death, and the veracity of statements from witnesses. The ratio may also aid the forensic scientist in interpreting the reliability of the alcohol concentration as determined from analysis of a blood sample or in estimating the blood alcohol concentration when only a vitreous humor sample is available.

RÉSUMÉ

Les concentrations sanguines post-mortem d'alcool ont été comparées avec celles de l'humeur vitreuse et de l'urine. L'étude portant sur les histoires de 182 cas établit l'influence de l'ingurgitation récente et l'état de conscience avant le décès sur les rapports des concentrations d'alcool humeur vitreuse/sang et urine/sang.

Les valeurs moyennes et déviations standards pour trois différentes catégories sont: non buveurs/conscients(1.26 ± 0.11); non buveurs/inconscients(1.46 ± 0.14); buveurs/conscients (0.91 ± 0.17). La catégorie “non buveur” se réfère à un sujet qui était possiblement au stade post-absorption avant l'incident. Les rapports UR/BL tendent à confirmer l'interprétation des résultats VH/BL. Les rapports VH/BL pourraient donner à l'enquêteur d'importants éléments concernant les habitudes de consommation d'alcool de la victime, son état de conscience précédant sa mort et la véracité des déclarations des témoins. Ce rapport pourrait aussi aider le toxicologue judiciaire à interpréter la concentration sanguine d'alcool telle que mesurée à partir d'un échantillon de sang ou de l'estimer plus facilement lorsque seule l'humeur vitreuse est disponible.
