Hydroxyzine and Possible Metabolites

ABSTRACT

This paper reports a fatal case of hydroxyzine poisoning and explores the question of possible metabolites of this drug, one of which is identified as norchlorcyclizine. Results from the present case are compared to those from three other cases in which hydroxyzine was ingested but in which death was not due to hydroxyzine poisoning. This comparison suggests that postmortem redistribution may occur with hydroxyzine. It is further suggested that the ratio of norchlorcyclizine to hydroxyzine may help to identify the occurrence of postmortem redistribution as well as to distinguish acute and chronic dosing with the drug.

RéSUMÉ

Une enquête de décès résultant d'une surdose d'hydroxyzine est présentée. La question portant sur l'identification de métabolites possibles de cette drogue incluant la norchlorcyclizine est examinée. Les résultats de cette enquête sont comparés à ceux obtenus dans trois autres cas lorsqu'il y avait eu une ingestion d'hydroxyzine sans pour autant résulter en un empoisonnement mortel. Cette comparaison semble démontrer qu'une redistribution postmortem d'hydroxyzine est possible. De plus, il est suggéré que le rapport des concentrations de norchlorcyclizine et d'hydroxyzine peut aider à élucider une redistribution postmortem ainsi qu'à distinguer une administration isolée d'une administration chronique.
