Severe Meprobamate Intoxication: In Death as in Life?

ABSTRACT

An unusual case is presented where a toxicological investigation conducted in one laboratory was important for the proper interpretation of postmortem toxicology findings in the same person, in a different laboratory, seven months later. The first incident, a minor motor vehicle accident, involved a severe meprobamate intoxication in which the person's blood concentration was 98 mg/L. In the second incident, a fatal house fire, the postmortem blood meprobamate concentration was 96 mg/L. Knowledge of the first toxicological investigation and circumstances surrounding the matter aided interpretation of the postmortem toxicology findings: that the very high blood meprobamate concentrations probably resulted from chronic abuse of the drug and not from a suicidal overdose.

Un cas inhabituel est présenté où une enquête toxicologique d'un certain laboratoire était importante pour l'interprétation appropriée des découvertes toxicologiques rapportées par un autre laboratoire, sept mois plus tard, pour la même personne mais cette fois-ci, après la mort. Le premier incident était un accident mineur de la route et une concentration sanguine de méprobamate chez le conducteur a été trouvée. Le deuxième incident était un incendie mortel à la maison impliquant une victime dont la concentration sanguine postmortem de méprobamate était 96 mg/L. La connaissance des découvertes de la première enquête toxicologique et des circonstances qui s'y rattachaient ont aidé à interpréter les résultats postmortem toxicologiques de la deuxième enquête. Les concentrations sanguines très élevées de méprobamate ont tout probablement résulté d'un abus chronique de cette drogue et non pas d'une surdose du type suicidaire.
