ABSTRACT
Many survivors of acute respiratory failure suffer from mood disorders following discharge from the hospital. We investigated the feasibility of intensivists delivering psychological support based on positive suggestion (PSBPS) to 20 intubated patients to reduce their psychological distress. Thirteen patients completed follow-up surveys. Of those, 9 remembered the intensive care unit physician talking to them, and 7 described it as comforting. Five patients (38%) met criteria for anxiety, depression, and acute stress. In comparison to historical controls, intervention may be associated with lower estimated odds of anxiety. PSBPS can be performed with patients in parallel with medical treatment to potentially reduce psychological morbidity and to humanize critical care. A larger randomized study is warranted to assess the efficacy of PSBPS.

ZUSAMMENFASSUNG Viele Überlebende eines akuten Atmungsversagens leiden nach Entlassung aus dem Krankenhaus an einer affektiven Störung. Wir untersuchten die Möglichkeit psychologischer Unterstützung durch die Intensivärzte, basierend auf positiver Suggestion (PSBPS) an 20 intubierten Patienten, um deren psychologischen Stress zu reduzieren. 13 Patienten schlossen die follow-up Untersuchungen ab. Davon erinnerten 9 Patienten, wie der Arzt der Intensivversorgung mit ihnen sprach, und 7 Patienten beschrieben das als wohltuend. 5 Patienten (38%) erfüllten die Kriterien für Angst, Depression und akuten Stress. Verglichen mit Kontrollen aus der Vergangenheit war die Intervention mit weniger Angst verbunden. PSBPS kann bei Patienten parallel zur medizinischen Behandlung durchgeführt werden, ummöglicherweise das psychologische Leiden zu lindern und die kritische Versorgung menschlicher zu gestalten. Eine umfangreichere randomisierte Studie ist erforderlich um die Effizienz von PSBPS zu untersuchen.

Nous avons étudié la faisabilité d’intensivistes fournissant un soutien psychologique basé sur une suggestion positive (PSBPS) à 20 patients intubés pour réduire leur détresse psychologique. Treize patients ont rempli des enquêtes de suivi. Parmi ceux-ci, 9 se souvenaient que le médecin de l’unité de soins intensifs leur avait parlé et 7 l’ont décrit comme réconfortant. Cinq patients (38%) répondaient aux critères d’anxiété, de dépression et de stress aigu. Par rapport aux témoins historiques, l’intervention peut être associée à des cotes d’anxiété estimées plus faibles. Le PSBPS peut être réalisé avec des patients en parallèle avec un traitement médical pour potentiellement réduire la morbidité psychologique et humaniser les soins intensifs. Une étude randomisée plus importante est justifiée pour évaluer l’efficacité du PSBPS.

RESUMEN Muchos sobrevivientes de insuficiencia respiratoria aguda sufren de trastornos del estado de ánimo después de ser dados de alta en el hospital. Investigamos la viabilidad de ofrecer apoyo psicológico basado en sugerencias positivas (APBSP) a 20 pacientes intubados para reducir su aflicción psicológica. Trece pacientes completaron las encuestas de seguimiento. De esos, 9 recuerdan que el médico de la unidad de cuidados intensivos habló con ellos, y 7 lo describieron como reconfortante. Cinco pacientes (38%) alcanzaron los criterios para ansiedad, depresión y estrés agudo. En comparación con controles históricos, la intervención podría estar asociada con una menor probabilidad estimada de ansiedad. El APBSP puede implementarse de forma paralela al tratamiento médico para reducir morbilidad psicológica y humanizar el cuidado crítico. Se justifica un estudio aleatorizado más grande para evaluar la eficacia del APBSP.
