Abstract
Aneurysm of the circle of Willis is the most important cause of hemorrhage into the subarachnoid space and should be the primary diagnostic consideration in every case. Angiograms should be made promptly, but the neurosurgeon should see the patient first. Treatment is surgical if the lesion is accessible. The goal is to exclude it from the circulation, leaving the parent vessel intact. If this goal is unattainable, it may be necessary to accept a result which is less than ideal. Expedient compromises are discussed.
