The success of anticoagulant therapy for progressing strokes depends on the earliest possible treatment, but first strokes due to intracranial hemorrhage must be excluded. A guide for selecting stroke patients most likely to benefit from immediate anticoagulant therapy and an illustrative case are presented and discussed.
