Abstract
A giant air-filled bronchogenic cyst is an unusual finding in an adult. In the case presented here, initial therapy involved closed thoracotomy and chest intubation because of an erroneous diagnosis of pneumothorax, thereby delaying proper surgical treatment. Careful scrutiny of the costophrenic sulcus on chest x-ray would have shown the feature that differentiates bronchogenic cyst from the condition it so closely mimics.
