Abstract
Airway obstruction in COPD is primarily in the small peripheral airways, and the available evidence suggests that this is due to organic disease in these airways, as Laennec originally suggested. Airway resistance increases even more during expiration, apparently owing to dynamic compression of the large airways rather than to a check valve in the small airways.
