Recent observations correlating surgical findings with preoperative symptoms have shed new light on the pathophysiologic mechanisms of transient ischemic attacks and strokes. Drs Persson and Jewell explore the data that have led them to focus on intraplaque hemorrhage as the major culprit and suggest the implications of this conclusion for patient management.
