The advent of intradiscal nucleolysis, a technique whereby chemical dissolution of a protruding lumbar disk is attempted, evidences that our approach to the management of symptomatic disk herniation is changing. Certain patients are ideal candidates for this approach, and early results are most encouraging. Surgery can always be done if the procedure fails.
