What to do when conservative measures fail

Conservative treatment of gastroesophageal reflux is effective in 95% of cases. For those patients who remain symptomatic, however, antireflux surgery has provided relief in more than 90% of cases. The authors support their recommendation of posterior fixation of the lower esophageal sphincter, introduced by Dr Hill in 1966, with data from their experience at the Mason Clinic and provide evidence of improved success rates with the addition of intraoperative manometry.
