Changing demands in health care have stimulated the development of a new model of home care. At Park Nicollet Medical Center, Minneapolis, home care services are tailored to a specific diagnosis, a model which differs in important ways from traditional programs. The four programs currently functioning at Park Nicollet offer patients safe and cost-effective care at home rather than in the hospital.
