Identification of factitious hypoglycemia can be a lengthy, frustrating process for the physician. However, reliance on circumstantial evidence alone is no longer necessary to prove factitious disease. Drs Ensberg, Gossain, and Rovner discuss when to be suspicious of patients presenting with hypoglycemia, how to determine whether the disease is self-induced, and why successful treatment is difficult. Four case reports covering a wide spectrum of manifestations are included.
