Dr Chahine approaches the question of the role of coronary artery spasm by describing ischemic heart disease as a spectrum, with fixed atherosclerotic obstruction on one end and spasm on the other. Between the extremes, each mechanism of ischemia plays a greater or lesser role. Spasm's role in Prinzmetal's angina has recently been delineated, permitting ready identification and effective treatment, as described here.
