That asymptomatic carotid bruit is associated with increased mortality, especially from cardiac disease, and with risk of stroke now appears to be irrefutable. What can or should be done diagnostically and therapeutically is less clear-cut. In this article, Dr Gorelick poses five questions important to clinicians treating patients with carotid bruit and then proceeds to answer each, drawing on the latest information available.
