“Difficult” or “problem” patients are ubiquitous in medical and surgical practice. They typically behave in an unexpected, puzzling manner and present an enigmatic clinical picture. To many physicians, the treatment of these individuals seems like a Sisyphean task. Dr Fisch describes a subgroup of difficult patients—masochistic patients—and suggests guidelines for their recognition and therapy.
