Ankylosing spondylitis, by its limited prevalence, is considered a rare disorder. However, its classic presenting symptom, low back pain, is seen very commonly by physicians. Dr Lisse describes the clinical features and manifestations that distinguish this from other musculoskeletal disorders. He also discusses the controversial practice of testing for HLA-B27 and cautions against its use in routine care of patients with back pain. The prognosis he offers is promising, provided the patient receives appropriate and timely pharmacologic and physical therapy.
