Because early prostatic carcinoma is asymptomatic and many men avoid the routine rectal examination needed to detect it, metastasis is present at diagnosis in 40% of patients. The authors discuss monitoring the course of the disease with tumor markers. They also describe advantages and disadvantages of the four types of endocrine-ablation therapy as well as management of secondary manifestations.
