Because the prognosis with primary pulmonary hypertension is grim, all potentially treatable causes must be excluded before primary disease is diagnosed. Both invasive and noninvasive studies (eg, cardiac catheterization, pulmonary function tests) are necessary. Results of medical therapy have generally been discouraging; a few patients may benefit from heart and lung transplantation.
