New thoughts on the classic pyramid approach

The aim of traditional early treatment of rheumatoid arthritis has been to preserve joint function and thus quality of life by recommending range-of-motion exercises and use of anti-inflammatory agents. Ironically, this approach, which reserves the use of disease-modifying antirheumatic drugs until later in the disease course, may allow joint destruction to progress. The authors summarize a newer approach, which introduces these agents earlier in treatment.
