What course of action should you take when an elderly patient presents with persistent fever? In most cases, the fever can be explained by the presence of infection, connective tissue disorder, or neoplasia. Diagnosis is best achieved through a sequential approach in which subtle clues obtained on initial history taking and examination point to subsequent steps in evaluation. This approach is described by the authors, who also provide an algorithm and illustrative case.
