Dietary restrictions and diuretics are usually effective therapy for ascites in patients with cirrhosis. For some patients, however, ascites is truly refractory to treatment with diuretics and the prognosis is dismal: Less than 30% live as long as 1 year. Drs Porayko and Wiesner describe the proper implementation of diuretic therapy and discuss the treatment options that remain when it fails.
