Traditionally, enterococcal infections have been thought to be acquired through endogenous means, since enterococci are present in virtually everyone. However, exogenous and nosocomial spread has now been noted as well. Enterococci have the ability to acquire resistance by a variety of mechanisms, and the increased incidence of resistant strains is especially bothersome. The authors describe the synergistic combinations of broad-spectrum antimicrobials that are needed to control these infections.
