Abrupt deterioration in renal function often reflects proliferation of infectious organisms within the kidney tissue or the toxic effects of drugs used to treat such infections. Thus, acute interstitial nephritis has the confusing potential of being cured and caused by the same agent. Dr Fried provides clues to diagnosis and summarizes the many pathogens and pharmacologic agents that have been implicated in this disease.
