Myocardial failure is linked to the onset of congestive heart failure through pathophysiologic mechanisms that are not yet fully understood. Aldosterone, the body's most potent miner- alocorticoid hormone, contributes to intravascular and ex- travascular volume expansion and is a major determinant in the appearance of symptomatic failure. In this article, the authors emphasize the importance of secondary hyperaldosteronism in congestive heart failure and the potential benefits of antialdosterone therapy.
