Recurrent bacterial meningitis can be caused by acquired or congenital gross anatomic defects, foci of infection, or disorders in immune mechanisms. The diagnosis of occult sinus fracture can be a challenge and, as in the patient described here, requires a high index of suspicion when a child has a remote history of head injury and repeated episodes of meningitis.
