Severe, episodic pain in the rectum may prompt extensive— and expensive— diagnostic testing. If the cause is proctalgia fugax, such testing is unnecessary and wasteful. The author, a gastroenterologist, offers a guide to prompt recognition of the disorder based on patient history and suggests several therapeutic stategies that may help to relieve the pain.
