Exophiala jeanselmei causing mycetoma pedis in India

Abstract

Exophiala jeanselmei causing mycetoma pedis in a 65-year-old Bengalee businessman was described. The right lower leg and foot were irregularly swollen, firm, painless, non-tender and had many small nodules and sinuses all over the swelling. The lower end of the tibia and bones of the foot were affected. The discharge from the sinuses and nodules contained black brown, soft, vermicular, crescent-shaped to irregular granules, measuring 0·5–2·00 mm and composed mostly of swollen spherical cells, 4–8 μm diameter, and a few hyphae, 2·5–3·0 μm diameter. The isolate was polymorphic in nature and exhibited various morphological features such as yeast-like cells producing secondary conidia, toruloid hyphae, and terminal and intercalary annellophores bearing annelloconidia at one or more points.

Résumé

Observation d'un mycétome du pied dû à Exophiala jeanselmei chez un homme d'affaires Bengali de 65 ans. La partie inférieure de la jambe et le pied droit étaient irrégulièrement enflés, fermes, indolores; et toute la zone tuméfiée présentait de nombreux petits nodules et sinus. L'extrèmité inférieure du tibia et les os du pied étaient atteints. Des sinus et des nodules ś échappaient des grains brun-foncés, mous, vermiculaires, en forme de croissant ou irréguliers, ils measuraient de 0,5 à 2 mm et étaient principalement composés de cellules sphériques gonflées (4–8 μ). et de quelques hyphes (2,5–3,0 μ). La culture était de morphologie variés: cellules levuriformes, hyphes toruloides, cellules à aspect de annellophores, conidiophores supportant des conidies on plus d'un point.
