Intrinsic factor (IF) secretion was studied in 73 patients (with low or abolished acid secretion) who also had a gastric biopsy. The IF secretion declined significantly with increasing severity of the mucosal lesion. The mean values (in kilo-units/hour) after augmented histamine stimulation were: 10.6 (superficial gastritis); 7.9 (atrophic gastritis with acid secretion); 2.1 (hypo-achlor-hydria). In patients with pernicious anaemia the IF output in all cases was below 0.2, showing no overlap with achlorhydric patients without pernicious anaemia.
