A randomized controlled trial of truncal and selective vagotomy, each combined with a drainage procedure, was made in 81 patients with chronic duodenal ulcer. 76 patients were re-examined 5 years later. No difference in histamine- or insulin-activated gastric acid secretion could be demonstrated ten days after the operation. Five years later the insulin-activated secretion was higher after selective than after truncal vagotomy. At the 5-year follow-up no difference was found in recurrent ulcer rate, but diarrhoea was more frequent after truncal than after selective vagotomy. One patient was invalidated from diarrhoea. Dumping frequency and weight changes were equal in the two groups.
