Relationship between Serum Gastrin Concentrations and Gastric Acid Secretion in Duodenal Ulcer Patients before and after Selective and Highly Selective Vagotomy

Relations between serum gastrin concentrations measured by radioimmuno-assay and gastric acid secretion were investigated during insulin tests before and after operation in 17 duodenal ulcer patients treated with selective vagotomy and pyloroplasty and in 17 other duodenal ulcer patients treated with highly selective vagotomy without drainage. Augmented histamine tests were performed pre- and postoperatively and the results related to serum gastrin concentrations. The gastrin and acid response to hypoglycaemia were positively related before operation, but not after the two vagotomies, the acid response being reduced and the gastrin response unchanged. No relation was found between serum gastrin concentrations and histamine-activated acid secretion, either pre- or postoperatively.
