The significance of the collateral venous blood circulation in patients with extra-hepatic block of the portal trunk is described. The choice of surgical or non-surgical treatment of these patients is discussed. Patients treated conservatively have been followed for 2 to 4 years after their gastrointestinal bleeding episodes. Changes in portal tension and angiographic findings are demonstrated on the basis of repeated splenoportographies.
