A reliable comparison of the frequency of cholelithiasis in various geographical regions or at various periods of time can only be made if representative or unselected agc-sex-population groups are studied. We compared the frequency of cholelithiasis in 85 % residents of Prague 2 and Malmö over age 10 years who died in 1963–66. The total population at risk was 100,000 and 250,000 inhabitants respectively. Thus, our study was strictly population-related. The frequency of cholelithasis in each age-sex-town group was very close in higher agc-groups. The average frequency was practically identical in both towns – i.e. roughly 30 % in males and 50 % in females. We were able to demonstrate a positive correlation between the thickness of subcutaneous fat of the abdominal wall and the occurrence of gallstones. Moreover, a strong association between gallstones and cancer of the gallbladder was found. On the other hand, we did not see any correlation between cholelithiasis and aortic or coronary atherosclerotic lesions, between cholelithiasis and diabetes mellitus, pancreatic necrosis, and peptic ulcer.
